  • Always check with your physician to make sure you are using the appropriate forms for your situation.

Guidelines for Palliative Care

To Get The Guidelines Book at RH or LC

  • Go to the library site:  www.fordham.edu/library
     
  • Click Library in the "My Accounts" drop down menu.
     
  • Click on "Review My Account."
     
  • Log into "My Account" using your FIDN (without the preceeding "A") as the User ID, and follow the instructions for determining your PIN . . .  or use the default pin changeme.
     
  • Click on "Search/Home" to begin, and type your catalog search. Next, click on the title of the item you want.
     
  • Click on the "Place Hold" link on the left, select the pick-up library, and click on the "Place Hold" rectangle near the bottom to complete your transaction.
     
  • Click on "Logout" at the top right-hand corner to exit your account.
